7/23/2023 EnchantaVaughn Approved 1. Behavior: Leona exhibits a poor attitude when transitioning from social setting to therapeutic setting. Leona displayed hostile and disruptive manners toward members of her family. QMHP-C informed Leona session was going to focus on her anger. 2. Behavior: During the second session, Leona was upset with her mother evident by her rolling her eyes at her mother when discussing ways to calm herself down when she is angered. 1. Intervention: QMHP separated Leona from inappropriate family interactions, in efforts to reduce tension and stress-provoking situations in the home environment that seemed to start problems between her, her mom, and younger brother. QMHP encouraged Leona to resist negative behaviors in efforts to refrain from requiring to be redirected by her mom so many times. QMHP provided understanding of momâs perspective of her disciplinary actions and why she is there to help resolve any issues related to her mental health. 2. Intervention: QMHP supported Leona with working on herself and trying her best to understand the purpose of each session. QMHP reminded Leona of ISP objectives. QMHP reviewed examples of alternative appropriate behaviors she could engage in without being so disruptive. QMHP encouraged Leona to refrain from displaying negative behaviors towards her mom, to talk calmly, and be mindful of how she responds so that it is clear what her intentions are and where her heart is. 1. Response: Leona modeled how to separate herself from negative situations so that she doesnât continue to be the problem but a part of the solution. Leona understood the importance of listening and following directions and have reduced time spent outside to learn more about ways to handle her home obligations and support her family. Leona utilized relaxation strategies. Leona reported a decrease in symptoms following the relaxation techniques. 2. Response: Leona was acceptive of redirection given by her mom and counselor. Leona recognized her triggers and was able to identify common things that she used bother her. Leona was observed strategizing a plan to better herself by writing down her thoughts and feelings in a journal and exploring her creative side with arts and crafts. Leona was taught to take deep breaths until she no longer feels upset or the need to take her anger out on anyone else. 3.Response: Leona reflected on her weekly plans. Leona stated she didn't need help with anything at the moment. Leona communicated by smiling and exhibiting good eye contact. Leona was able to discuss with QMHP on how she will learn how to talk to her mother about things bothering her in the home when it comes to her siblings
